Family Care Partners
Family Care Partners provides flexible, personalized in-home support of your loved one when work, travel, or illness force changes to your routine, or when you find yourself emotionally or physically overwhelmed by your responsibilities as primary caregiver. If these feelings are all too familiar, Family Care Partners from HCR can help with a flexible, customized care plan overseen by a nursing professional.
Geriatric Care Management
Geriatric Care Management can be a valuable component of an overall Family Care Partners or Expanded Care solution, providing education and connections to needed community resources; guidance on long term planning, housing options, and alternative types of care; and consulting on costs and coverage associated with insurance options. We can also serve as the “eyes and ears" of family members who live far from their parent or loved one requiring care.
Lifelongevity
Lifelongevity is a unique program designed to help you regain and sustain a healthy, active life. From designing and guiding a personalized exercise program, to dietary planning, to falls risk assessments and falls prevention care plans, Lifelongevity will deliver a restorative and preventative wellness program tailor-made to meet your unique needs.
Expanded Care
Expanded Care is designed to complement those services covered by your insurance provider. With Expanded Care we are often able to increase the speed or extent of your recovery by increasing the frequency, duration, or scope of services beyond that which your insurer will pay for. Services have no time limit restrictions, and an aide is always available at the time most convenient for you.
