Family Care Partners provides flexible, personalized in-home support of your loved one when work, travel, or illness force changes to your routine, or when you find yourself emotionally or physically overwhelmed by your responsibilities as primary caregiver.

As a busy person responsible for an aging parent, chronically ill family member, or other loved one requiring ongoing care, companionship, or supervision, planning ahead for life’s unavoidable twists can give you peace of mind.  HCR’s Family Care Partners program and our staff of compassionate, trained and professional home care specialists will make sure your loved one’s needs are met, whether those needs are medical, social, or entail assistance with activities for daily living.

At HCR we realize that caregivers may sometimes reside in another town or even another state.  It’s in those instances where having complete trust and confidence in those caring for your aging parent or other loved one is particularly important. 

HCR can help you and your loved one with:

  • Coordination and transportation for medical appointments
  • Assistance and transportation with outings and functions

How is This Different from Other “Companion Care” Services?

With Family Care Partners from HCR, your loved one’s care is overseen by a professional nurse case manager, who in addition to coordinating the activities of each individual who may be involved in supporting your family member, will facilitate:

  • An assessment of your loved one’s condition and the identification of any medical needs, along with escalation to / consultation with your physician where applicable

  • Identification of any insurance-eligible needs,
    helping to preserve your loved one’s and your
    own financial resources

HCR has the necessary certifications and the flexibility to easily move your loved one from Medicare, Medicaid, or other insurance eligible care, to private-paid care and back again as your family member’s insurance eligibility status changes.  And you’ll retain the same team of devoted, professional caregivers the entire time, despite any changes in insurance coverage or eligibility.

Who Will be Caring for My Family Member?

In addition to the nurse case manager who guides and coordinates your loved one’s personal health team, you may be supported by a broad array of skilled home care specialists, depending on your own and your family member’s needs, including:
